Non-Insured Services Policy and Block Fees

The government of Ontario (OHIP) does not pay for all health care services. More and more services are becoming delisted (non-insured) and being left to the responsibility of the patient. Included in this list are services such as notes for work, telephone prescriptions, 3rd party requested services, camp forms, insurance reimbursement forms. All of these services are available by request of your physician. The time taken to complete these documents does take time, and as a result, there is a fee charged for the service.

The College of Physicians and Surgeons of Ontario and the Ontario Medical Association have set forth guidelines for the structuring of these services and fees as set out by the Divisional Courts of Ontario. As a result, the CHSM lists the following services that are available and the accompanying fees. One may choose to pay the block fee indicated which would cover a patient for the year rather than paying for each individual service as it arises. The fees indicated will cover the CHSM's expenses and the physician's time for the completion of the service provided. Please inquire at reception if you have any questions or concerns in regards to the services and the fees listed.

4. MVA Insurance Forms $ 75.00 (insurance coverage)
5. WCB forms WCB covered
6. Medical Chart Transfer $ 25.00 (+ $0.10/page)
7. Handicap Parking Forms $ 20.00
8. Insurance Reports (variable)
9. Medicolegal Reports to be determined (variable)
10. Block Fee for a full year $ 75.00 (for a family)
$ 50.00 (for an individual)
$ 25.00 (for a senior)


Non-Insured Services Policy and Block Fees

The Ontario Health Insurance Plan (O.H.I.P.) does not pay for all health care services. The term delisting is becoming a more familiar in the late '90's and will follow us into the 21st Century. Delisting refers to any service or expense that used to be covered by O.H.I.P. and is no longer provided for by the government. Most recently, the government has put a restriction on the number of O.H.I.P. therapy treatments that a patient is entitled to per year as provided for by the government. Each patient is entitled to 20 treatments per year in total regardless of the number of musculoskeletal conditions there are to be treated. If a patient requires more than 20 treatments, he/she is required to get approval from a specialist (a physician with the post nominals F.R.C.P. or F.R.C.S. after his/her name) before the treating facility will offer any further treatments. If specialist approval is not sought or received, then the service will then become delisted and the patient will be required to pay privately for the service. The fees for these services would then become the responsibility of the patient receiving the service. If you have any questions about this government policy, please ask from reception for any further clarifications.

If you would like a letter to be sent to your family physician, or other health care practitioner in addition to your family physician, then your family physician is required, out of courtesy, to refer you to one of C.H.S.M.'s physician's for a formal consultation and a detailed report will be sent to your physician without charge to the patient. If a formal consultation is not arranged, then a fee will be assessed to the patient for this service.
